Conditions of Service

To ensure we provide exceptional service, we kindly ask all clients to adhere to the following conditions. If you have any questions, please consult your local Belong Healthcare Practice Manager.

Service Agreement: Upon starting services, you’ll be asked to sign an agreement confirming your consent for our team at Belong Healthcare to provide services to you or your child. This agreement also outlines how the service appointments will be funded.

• Minors: For clients under the age of 18, we require parents or guardians to remain on the premises during therapy sessions, unless a different arrangement has been confirmed with your therapist.
• Smoke-Free Environment: Belong Healthcare is a smoke-free environment. We ask that you refrain from smoking while our therapists are providing services, including during home visits.
• Notification of Changes: Please keep us informed of any changes in circumstances that might impact the delivery of our services.
• Confidentiality: Maintain the confidentiality and privacy of staff and other clients, especially when participating in group programs.
• Respect and Dignity: We expect all staff members of Belong Healthcare to be treated with the utmost dignity and respect.
